The damage increase is proportionate to 1% of a damage increase per 1% of HP lost. To get a 30% damage increase, you have to be at 70% HP or below.
The skill hits in a slanted rectangle roughly matching the overall shape of the area the 'Void Splitter' slashes appear in. This means it can hit behind you and up to 2 platforms above you, reaching less high further ahead of you. The Void Explosion also hits within this same area.
